Avenidas Rose Kleiner Center provides a safe, therapeutic, and supportive environment for adults with cognitive and physical challenges. Caregiver support resources and services are an integral part of our program.

Our day-caring program promotes and encourages independence and provides stimulating activities in a comfortable and caring environment. Our participants enjoy many activities and therapeutic interventions:

  • Structured enrichment activities at various levels of cognitive functioning

Nurse Navigator Program

Our Nurse Navigator Program is part of the statewide Community Based Health Home Project (CBHH). 

The goal of this program is to improve the quality of life for participants allowing them to live in their own homes, and avoid admission to emergency rooms, hospitals, and skilled nursing facilities. 

Specializing in Care Management, our Nurse Navigator works closely with the entire care team, providing oversight, advocacy, and an extra layer of support for participants with complex medical issues and few social support systems.

Avenidas Rose Kleiner Center Aides on What Draws Them to This Work

On any given day at Avenidas Rose Kleiner Center, you see a flurry of activity – watercolor painting on one side of the room and noodle hockey on the other side of the room – and the group of individuals who orchestrate it all are the aides: Dina Caceres, Katherine Dubon, Mary Medina, Eileen Melton, Polly Peterson, Andrea Sahai, and Mercy Udoh.

We asked aides, “What draws you to this work?” The answers surprised and touched us.

Seriously, we have a lot of fun while keeping people safe and smiling, whether it’s music and crafts or bingo and gardening. Frequently Asked Questions

Fees are paid in three ways: with Medi-Cal approval, the daily fee can be covered by the participants’ Medi-Cal benefit; the VA will cover the fee for qualified veterans; and for those who pay privately, the fee is determined on a sliding scale according to income and assets. Private Pay fees range from $90 per day to $160 per day. The cost includes the program services, lunch, and transportation.

Participants can come two days up to five days a week, and the program lasts from 9:30 am until 2:30 pm. Participants choose the activities they want to engage in during the day and they may also receive physical, occupational and/or speech therapy, nursing services, or meet with a social worker. A delicious and nutritious hot lunch is served every day in a welcoming group environment.

Caregivers are very important to us. We offer a monthly Caregiver Support Group to give family members the opportunity to share the ups and downs of caregiving with other caregivers. The group meets on the third Wednesday of the month from 1 to 2 pm.

Staff members are always available for consultation and support. Social work can refer caregivers to additional resources for care and support when needed.
