Reopening Plans and Timeline
In June, the Avenidas Rose Kleiner staff will begin evaluating potential participants to determine an appropriate plan of care for them from each of our different disciplines, including Physical Therapy, Occupational Therapy, Speech Therapy, Nursing, Social Work Services and Activities.
On July 1, we will open our doors and once again, start providing our quality day-caring!
On August 28, from 10am to 2pm, we’ll be hosting a “Re-Opening Celebration Party” so you can come see all the improvements we’ve made to our center. You can enjoy a tour of our facility, refreshments, and the chance to win a door prize! RSVP to Susan Lam at .
As our center tends to serve older adults who are more at risk in the community, our team of professionals is excited to work with our participants in person again and help to improve their quality of life—and that of their caregivers!
Safety is our top priority as we transition back to our center. By the time we re-open, all staff and participants who are planning on returning to the center will be fully vaccinated.
Additionally, participants and staff will wear personal protective equipment such as masks, while tables will be physically distanced and equipped with sneeze guards to further mitigate any risk of exposure. When possible, we will provide opportunities for programming on our back courtyard.
This past year was hard on everyone, but especially those who have had limited opportunities to socialize and move around, so we are anxious to welcome them back!

Nurse Navigator Program
Our Nurse Navigator Program is part of the statewide Community Based Health Home Project (CBHH).
The goal of this program is to improve the quality of life for participants allowing them to live in their own homes, and avoid admission to emergency rooms, hospitals, and skilled nursing facilities.
Specializing in Care Management, our Nurse Navigator works closely with the entire care team, providing oversight, advocacy, and an extra layer of support for participants with complex medical issues and few social support systems.
