What the Document Covers
This document is a straightforward one-page Advance Directive that lets doctors and family members know what you want in the event you become ill, specifically
- Whether and at what point you want to be moved to a medical facility.
- How much sedation and pain relief you want and how alert you want to be.
- Whether you want to be put on a ventilator or other life support if you become sick enough to need it.
As with any other form of advance directive, it’s a good idea to discuss it with your loved ones so they’re aware of what you want.
How to Use This Document
You can complete the document either on your computer or by printing it. Take a picture of the signed document with your phone and send it to your doctor and your family members. Post it on your fridge so it’s handy if you need to call 911.
The document does not specify a person you want to make other medical decisions in case you can’t, so it’s not a substitute for your existing Advance Directive.
