Avenidas Rose Kleiner Center
The Avenidas Rose Kleiner Center in Mountain View is a supportive, caring adult day health program that encourages the vitality and well-being of less-independent older adults, enabling them to stay at home and avoid premature institutionalization. It also offers much needed respite for families. In this professional yet warm setting, adults living with a chronic disease, dementia, or a disability receive individualized therapeutic, social, and health services for some part of the day. Avenidas Rose Kleiner Center is licensed by the State of California to provide two levels of daytime care—adult day care and adult day health care.
Once enrolled, your loved one will benefit from:
- Social activities—interaction with other participants in planned activities appropriate for their conditions
- Transportation—door-to-door service in wheelchair accessible vans
- Meals—participants are provided with meals and those with special dietary needs are offered special meals
- Personal care—help with toileting, grooming, eating and other personal activities of daily living
- Therapeutic activities—exercise and mental interaction for all participants
I like all the activities here, especially the music program and the Hope discussion group run by the social worker.
Brain fitness games
Dad will brag to visitors that before he started going to the Avenidas Rose Kleiner Center, he
couldn’t even bend over and touch his knees, but now he can almost touch his toes, and he’ll prove it. He’s even lost 18 pounds since enrolling.
What does it cost?
Fees are paid in three ways: with Medi-Cal approval, the daily fee can be covered by the participants' Medi-Cal benefit; the VA will cover the fee for qualified veterans; and for those who pay privately, the fee is determined on a sliding scale according to income and assets. Private Pay fees range from $80 per day to $150 per day. For adult day health care participants, the daily fee covers all of the services the Center provides; For adult day care participants, there is an extra charge for transportation services if transportation is provided by the program.
What will my loved one be doing all day at the Center?
Participants can come two days up to five days a week, and the program lasts from 9:30am until 2:30pm. Participants choose the activities they want to engage in during the day and they may also receive physical, occupational and/or speech therapy, nursing services, or meet with a social worker. A tasty hot lunch is served every day in a welcoming group environment.
Is there any help for the caregiver?
Caregivers are very important to us. We offer a twice-monthly support group called "Family Group" to give family members the opportunity to share the ups and downs of caregiving with other caregivers. Staff members are always available for consultation and support. Social work can refer caregivers to additional resources for care and support when needed.
