COVID-19 vaccinations are only available to those who meet federal and state criteria. Currently, the County of Santa Clara is able to vaccinate healthcare personnel who live or work in Santa Clara County and those aged 65 or above who live in Santa Clara County.
COVID-19 vaccines are available at no cost, but they are only available to eligible individuals. If you do not meet eligibility criteria, you will not be able to receive your vaccine at your scheduled time.
You will be asked to sign an attestation at the time of your vaccine ensuring that you meet criteria. You will need to bring evidence of your eligibility, such as a photo ID showing your date of birth with age over 65.

If you are a patient of Kaiser, Palo Alto Medical Foundation, or Stanford Healthcare, you MUST schedule your vaccine appointment through their system.
Kaiser Permanente
If you are a Kaiser member over 65 or a healthcare worker, call 866-454-8855 to schedule an appointment.
Palo Alto Medical Foundation
If you receive primary care through PAMF and are over 65 or a healthcare worker, schedule through My Health Online or call (844) 987-6115.
Stanford Healthcare
If you receive primary care from Stanford Healthcare and are over 65 or a healthcare worker, schedule through MyHealth.
If you don’t receive healthcare from these systems or if you are a patient of Santa Clara Valley Medical Center, fill out the attestation form below to proceed with the scheduling process.
