- Comprehensive Assessment—A geriatric professional from our staff will visit your family member’s residence to assess physical, psychological, and social needs and capabilities.
- Assessment and Recommendations Report—We will prepare a comprehensive written report detailing observations and outlining the specific steps, services, and programs that will best address your family member’s individual needs and concerns
- Service Coordination—Once you have reviewed our report, we can assist in locating providers, facilitating interviews, and providing complete service coordination and management on an ongoing basis.
- Monitoring and Follow-up—When services are in place, we will visit in person or by phone to ensure that all services are professionally delivered. We will also keep you informed and updated on any issues that require your attention.
